Victims of Human Trafficking

Jeannie Baker Willhite and her team, have always supported those affected by human trafficking, and in an effort to continue assisting any individual who has been a victim, we provide you with the following process to remove adverse items that resulted from human trafficking.

1) Please Provide Proof of Identity

You may provide copies of a government-issued ID, driver’s license, birth certificate, or passport.

2) Victim Determination Document:

A document that determines a consumer is a victim of trafficking, made by either:

    • A Federal, State, or Tribal governmental entity
    • A court of competent jurisdiction, which includes certain documents filed in court
    • A non-governmental organization or human trafficking task force, including victim service providers affiliated with these entities, authorized by a Federal, State, or Tribal government entity
    • A self-attestation by a consumer who identifies as a victim of trafficking that is signed or certified by a Federal, State, or Tribal government entity, court of competent jurisdiction, or an authorized representative of these entities

3) Identified Adverse Items of Information

Identify the specific items of adverse information that should not be furnished by a consumer reporting agency. This information should only be charges that resulted from human trafficking.

4) Preferred Method of Contact

Indicate whether you want to be contacted via mail or e-mail in order to fulfill your request and provide the preferred contact information (e.g., mailing address, email address).
